Hybrid workflows — whether used in the transition from paper to electronic records or as workarounds to the electronic system — may cause a variety of errors.       

Through the Authority’s collaboration project and an assessment tool, Pennsylvania nursing homes successfully implemented infection control best practices and achieved decreases in infection rates.
